In order to be listed on the Xtreme Lashes® "Find A Professional" directory, you must:

Successfully complete the Xtreme Lashes® Training and Certification Workshop.
Have qualified for and received your Level One Xtreme Lashes® Certificate.
Satisfy the requirements for listing on the Lash Stylist Directory.
Download and fill out the form below and submit it via US MAIL along with your photos for review.

Xtreme Lashes® must reserve the right to remove your listing at anytime for any reason. For example, if we get reports of unsatisfied clients of yours, we may remove your name. We will usually attempt to discuss the situation with you prior to removing your name.

Once you have completed our Workshop and have completed at least 10 successful applications with satisfied clients, we welcome you to send in the form above and request your listing.
