Waiver Forms (Pad of 50):
Xtreme Lashes® feels it is vitally important to protect your interests by
having your clients sign a Waiver and Release form prior to each and every
eyelash extension application. We have spent a considerable amount of time and
research creating this valuable form for you. Our form includes acknowledgement
of risk statements as well as a section confirming the client’s knowledge of
how to properly care for their lashes. We recommend that you use these forms on
both new and repeat clients each and every time for the best possible
protection.
Note: Xtreme Lashes® highly recommends that you have your customers sign a
waiver before your beginning the lash application procedure. We have created
this waiver form for you to use with your customers should you decide to do so.
If you have any questions or doubts about the legality of using this waiver
form or about the protection a waiver may provide, please consult with an
